These terms are an agreement between you and Smart IT US Inc. (“we”, “us”) about the forwell app and the forwell.app website. By using forwell you accept them. If you do not accept them, do not use forwell.
forwell is a personal tracking tool. It is not medical advice, not a diagnosis, and not a medical device.
Nothing in forwell — including the cycle map, the estimated level curve, targets, insights or any report you export — is a recommendation about whether to take a medication, when to take it, or in what dose. Those decisions belong to you and the clinician who prescribed your treatment.
The estimated level curve is a calculation based on the doses you entered and a published half-life for the substance. It is an estimate drawn from your own history, not a measurement of what is in your body and not a forecast of how you will feel. Where we cannot make an honest estimate — for example when a compounded vial’s concentration is unknown, or when a substance has no published half-life — we do not draw one.
Patterns forwell shows you are patterns in what you logged. They can be incomplete or misleading, particularly early on, and they are never a substitute for medical judgement.
If you think you are having a medical emergency, call your local emergency number. Do not wait, and do not rely on forwell.
You must be at least 18 years old and able to enter into a binding agreement. forwell is offered for personal, non-commercial use.
You can use forwell without creating an account. If you create one, keep your credentials to yourself; you are responsible for activity under your account. You can delete your account from inside the app.
forwell offers a free tier and a paid tier, forwell Pro. Paid plans are auto-renewable subscriptions sold through the App Store.

Prices are in US dollars and may differ in your region and currency; the price shown to you at purchase is the price that applies. We may change prices for future periods and will tell you before a change takes effect.
Payment is charged to your Apple account at confirmation of purchase. Your subscription renews automatically unless you turn off auto-renewal at least 24 hours before the end of the current period. Your account is charged for renewal within 24 hours before the period ends.
You manage and cancel your subscription in your Apple account settings, not in forwell. Deleting the app does not cancel a subscription.
New subscribers may be offered a 7-day free trial. Unless you cancel at least 24 hours before it ends, the trial converts into a paid subscription at the price shown when you signed up, and the charge is taken automatically. A free trial is available once per subscription group; if you have used one before, Apple may not offer another. Any unused part of a trial is forfeited if you buy a subscription before it ends.
Purchases are processed by Apple, so refunds are handled under Apple’s policy and requested through Apple. We cannot issue refunds for App Store purchases ourselves, though we will help you where we can.
The logs you create — doses, symptoms, weight, photos, meals — are stored on your device, not on our servers. That has a consequence worth stating plainly: if you lose or reset your phone, or delete the app, and you have no backup or export, the data is gone and we cannot recover it. forwell gives you a backup to your own iCloud account and an export you can keep; using them is your responsibility.

Do not use forwell to break the law, do not try to reverse-engineer, disrupt or gain unauthorised access to it, and do not resell it or present its output as clinical advice to others.
forwell, its name, design and software are ours and are protected by intellectual property law. You get a personal, non-transferable, revocable licence to use the app. What you log is yours; we claim no ownership of it.
We may change, suspend or discontinue features. If we make a change that materially reduces what a paid subscription gives you, we will tell you in the app in advance. We may update these terms; when the update matters, we will say so before it takes effect.
You can stop using forwell at any time and delete your account in the app. We may suspend or end access if you break these terms. Ending the agreement does not by itself cancel a subscription bought through Apple — cancel that in your Apple account.
forwell is provided “as is” and “as available”. To the extent the law allows, we disclaim implied warranties of merchantability, fitness for a particular purpose and non-infringement. We do not warrant that the app will be uninterrupted or error-free, or that estimates and patterns it shows will be accurate for you.
To the extent the law allows, we are not liable for indirect, incidental, special or consequential damages, or for lost data or lost profits. Our total liability relating to forwell is limited to the amount you paid us in the twelve months before the claim. Nothing here limits liability that cannot be limited by law, including liability for death or personal injury caused by negligence, or for fraud.
These terms are governed by the laws of the State of Delaware, United States, without regard to conflict-of-law rules. If you are a consumer in the EU or the UK, you keep the protections of the mandatory law of your country of residence.
