Questionnaire Privacy Notice

This notice is about the Forwell questionnaire — the short set of questions that shows how Forwell could work for you. It adds to our Privacy Policy and Cookie Policy. Where they differ about the questionnaire, this notice controls.

1. Why we ask these questions

Forwell is a tracker for people taking GLP-1 medications. The questionnaire shows you how Forwell could line up your shots, symptoms, food and progress around the way your weeks actually go. You don't have to take it to download the app.

2. What we ask

Up to ten questions, plus one optional screen:

• where you are with your GLP-1 treatment — for example, about to start, in your first weeks, adjusting the dose, on maintenance, taking a break, or not on a GLP-1 yet;
• which medication you take;
• one question that matches the ad you came from — for example your goal, the symptoms you've had, or how you keep your shots on schedule;
• which symptoms you've had since starting;
• which day after your shot hits hardest;
• what's hardest about eating on your medication;
• how you keep track today;
• whether a doctor or prescriber has asked you to keep track of anything;
• what matters most to you right now;
• your age range;
• optional: your current and goal weight, or skip.

Answers are chosen from fixed options. Some questions are skipped depending on your earlier answers — for example, if you're not on a GLP-1 yet, we don't ask about symptoms.

Under privacy laws, these answers can be "consumer health data" (Washington My Health My Data Act and similar state laws), "sensitive personal information" (California) and sensitive or health information (Canada, Australia, New Zealand). We handle them with the protections those laws require.

3. Your consent

On the first screen, before any question, you tick an unticked box that reads:

The box is never pre-ticked, and Start doesn't work until you tick it. This is your consent to collect and use your answers for the questionnaire, and your confirmation that you are 18 or older.

Stopping. You can close the questionnaire at any time. On forwell.app nothing is kept once you close the tab. On the FunnelFox pages, email legal@smart-it.io if you'd like the answers you already gave deleted.

Sharing is separate. We don't share or sell your answers, and we would not do so without asking you separately first.

4. How we use your answers

• to choose the next questions and show you how Forwell could work for you at the end;
• on the FunnelFox pages, to see in aggregate how many people reach each screen, so we can improve the questionnaire.

We do not use your answers for advertising, send them to Meta, Google or any other ad platform, build retargeting, lookalike or health-interest audiences from them, or make any automated decision about you with legal or similarly significant effects.

5. Where your answers go

On forwell.app

Your answers are held in your browser's session storage for that tab, used only to show you the next screens, and erased when you close the tab. They are not sent to us, to Meta or to anyone else.

On the pages hosted by FunnelFox

We also run the questionnaire on pages hosted for us at get.forwell.app by FunnelFox, operated by Adapty Tech Inc. (United States), our service provider under a data-processing agreement.

• Your answers go to FunnelFox as you answer — that is how these pages work. FunnelFox uses them only to run the questionnaire for us — never for its own purposes, marketing or to train its systems.
• Within one hour of receiving them, FunnelFox anonymises or pseudonymises the details that identify you — such as your email address, device identifiers and IP address. It may keep them longer only if we ask in writing.
• Any onward hosting of health-related answers is limited to Amazon Web Services and OVH US.
• We don't receive your answers. FunnelFox gives us statistics about how many people reached each screen, not individual answers.

In neither version

Your answers are not carried into the Forwell app, not sent to any AI service, and not sent to Meta or any other advertiser.

6. Ad measurement on the questionnaire pages

Both versions of the questionnaire, like the rest of our web pages, load the Meta Pixel when the page opens. It tells Meta which pages you viewed and how far you got — never your answers. Tap No thanks on the cookie bar, or send a Global Privacy Control signal, to stop it. On forwell.app, if you visit from outside the United States or from Washington, Nevada or Connecticut, it loads only after you tap OK. Details: Cookie Policy and Privacy Policy, Section 4.7.

7. Your rights

You can ask us at any time to see, correct or delete the answers you gave on the FunnelFox pages, and you can withdraw your consent by closing the questionnaire and asking us to delete what you already answered. In California you can also limit the use of sensitive information, and opt out of the "sharing" of the measurement data in Section 6. 8. Not medical advice

What the questionnaire shows you is a reflection of what you told us — not a diagnosis, a prediction, or advice about whether, when or how much medication to take. Forwell is a tracker, not a medical device. Talk to your doctor or prescriber about your treatment. See the Medical Disclaimer.

9. Adults only (18+)

The questionnaire is for adults 18 and over, which you confirm in the consent box. We don't knowingly collect answers from anyone under 18. If we learn that someone under 18 took the questionnaire on the FunnelFox pages, we will have their answers deleted.
