At a glance

This policy covers our web pages — forwell.app and the questionnaire pages hosted for us by FunnelFox at get.forwell.app. The Forwell iPhone app doesn't use browser cookies; see our Privacy Policy.
We use one kind of optional cookie: ad measurement by Meta. We don't use Google Analytics, Google Ads or any other analytics or advertising cookies.
It starts when the page opens. Tap No thanks on the cookie bar, or send a Global Privacy Control signal, and it stops. On forwell.app, if you visit from outside the United States or from Washington, Nevada or Connecticut, it starts only after you tap OK.
Your answers never go into cookies or to Meta. Meta learns which pages you viewed and how far you got in the questionnaire — never your answers, your medication, your symptoms, your weight or your email.

1. What this policy covers

This Cookie Policy explains which cookies and similar technologies our web pages use, why, and how you can control them:

forwell.app — our website, including the landing pages and the questionnaire at forwell.app/start;
get.forwell.app — the questionnaire pages hosted for us by FunnelFox (Adapty Tech Inc.), our questionnaire provider.

"Cookies" here means cookies and similar technologies — pixels, tags, and your browser's local and session storage.

It does not cover the Forwell iPhone app. The app doesn't use browser cookies; what it collects is described in our Privacy Policy.

2. Cookies we use

2.1 Strictly necessary (always on)

Needed for the pages to work and to remember your choice. You can't turn these off without breaking the pages.

2.2 Advertising measurement (optional)

Lets us see which of our ads on Facebook and Instagram lead people to Forwell, and helps Meta show our ads to people likely to find them useful.

What the pixel tells Meta: that you viewed a page, started the questionnaire, reached its steps and reached its last screen, together with your browser and device details and IP address.

Hard limit. Meta never receives your questionnaire answers, your medication, symptoms or weight, your email address or other contact details, or anything from the app. We don't build retargeting, lookalike or health-interest audiences from our visitors, and we never upload email or customer lists to an ad platform.

3. Your choices

We'd rather describe this exactly than in a friendlier but looser way. The Meta Pixel loads when the page opens, and the cookie bar lets you stop it:

No thanks — our pages stop sending events to Meta from that moment, and on later pages the pixel doesn't load.
OK — ad measurement continues as described in Section 2.2.
Outside the United States, and in Washington, Nevada and Connecticut. On forwell.app, if your connection shows that you're visiting from outside the United States or from one of these three states — or if we can't tell where you are — the pixel doesn't load until you tap OK.

Your choice is remembered in your browser (fw_consent). Change it at any time with Cookie choices or Do Not Sell or Share My Personal Information at the bottom of our pages.

3.2 Global Privacy Control (GPC)

If your browser sends a Global Privacy Control signal, we treat it as No thanks: the Meta Pixel doesn't load on our pages at all. Under California law and similar state laws, this is a valid request to opt out of the "sale" or "sharing" of your personal information for advertising. We don't sell personal information.

3.3 Your browser and Meta

You can also delete or block cookies in your browser settings (Safari: Settings → Safari → Advanced → Website Data; Chrome: Settings → Privacy and security → Cookies). Blocking strictly necessary storage may stop the questionnaire from working.

You can control the ads Meta shows you in your Facebook or Instagram ad settings: facebook.com/adpreferences. General US opt-out tools: optout.aboutads.info and optout.networkadvertising.org.

4. Do Not Track

There is no common standard for browser "Do Not Track" signals, so we don't respond to them. We do honour Global Privacy Control (Section 3.2).

5. The Forwell app

The Forwell iPhone app doesn't use browser cookies and has no advertising trackers. If you install it after tapping one of our ads, Apple's SKAdNetwork — not us — may tell Meta that the ad led to an install, without identifying you. Usage analytics in the app is off until you turn it on. Details: Privacy Policy, Sections 4.1 and 4.8.

6. Your privacy rights

How we handle the information these cookies collect, and your rights in your state or country — including opting out of "sharing" and limiting the use of sensitive information — are in our Privacy Policy, Sections 9 and 13–16.

7. EEA and UK

Forwell is not offered in the European Economic Area or the United Kingdom. If that changes, we will load optional cookies there only after prior opt-in consent, as those laws require.

8. Changes

When we add, remove or change cookies, we update this policy and the Effective Date above, and ask you again where the law requires it.
